Job Description:

The Behavioral Health Support Specialist works under the supervision of the Behavioral Health Director to complete BH referrals internally and externally for patients. This position will also provide support to Behavioral Health Department personnel and coordinate certain aspects of departmental tasks with the Nursing and Clinical Support Services Supervisor, as well as Quality Management and Compliance Department personnel.

 

DUTIES PERFORMED:

  • Demonstrates a working knowledge of the EHR, clerical and referral processes, including insurance authorizations.
  • Responds professionally and positively to staff, patients and community contacts.
  • Respond to patients and others in a calm, compassionate way.
  • Identify intended provider that best suits patient needs - may require reviewing medical record, beyond referral.
  • Establishes and maintains effective working relationships with all members of the health care team and demonstrates appropriate communication skills.
  • Accepts patient-related information from various sources and ensures reports are immediately forwarded to the appropriate party.
  • Process practitioner referral orders in a timely fashion and forwards necessary EHR information required for each individual order.
  • Schedules In-House referrals as needed and follows established processes and guidelines.
  • Schedules External referrals for CURRENT behavioral health patients, as needed for Behavioral Health department, in collaboration with Behavioral Health Care Advocate.
  • Obtains necessary referral pre-authorizations.
  • Addresses STAT referrals same day, and/or as occurring, in conjunction with Care Advocate/ Behavioral Health staff. Communicates all "stat" reports/issues to appropriate team member (LISW, PCP, Behavioral Health Director).
  • Problem solve issues with referrals, with assistance of Behavioral Health Director, LISW or other staff.
  • Understands the purpose of and has a working knowledge of Patient Centered Medical Home concepts.
  • Tracking: works with Task Management, as needed, to track outcomes for referrals, hospitalizations, and other required information supporting Patient Centered Medical Home guidelines. This will require team work with the Behavioral Health Care Advocate as well.
  • Contributes constructive ideas for a more efficient Behavioral Health referrals process.
  • Participate in training to expand knowledge of common behavioral health issues such as SUD, Mood Disorders, and Chronic Pain Management.

Education:

High School Diploma or GED equivalent.

Customer Service Experience

 

Preferred:

1 - 2 Years Higher Education in related health, human service, or social work fields

Electronic Health Record Experience

  • Must have working knowledge of computer programs and professional construction of emails
  • Must demonstrate
