Care Advocate for the Medical Respite HCH Program

Job Description:

Details:

• The Care Advocate will provide needs self-assessments, client/community advocacy information, health information, and referral to the health center.  The Care Advocate will work with patients requiring assistance to obtain medications by helping with the forms required by pharmaceutical companies.  This position will provide patient assistance with accessing other health and social support resources.

Experience and Skills:

Education:

• Minimum: Bachelor’s in Social Work, LBSW preferred
• BS in Psychology, Human Services or related field

Knowledge and Skills:

• Must have a working knowledge of computer programs and professional construction of emails.
• Must demonstrate excellent communication skills, spelling, legible handwriting,  ability to accept responsibility, being a motivator and enthusiastic learner, and ability to work under stress and with others
• Able to be an effective client advocate.  Ability to help clients understand and prioritize individual and family needs
• Must have a working knowledge of the community health care system and related supportive resources
• Must have the ability to understand Medicaid eligibility requirements and the certification/recertification process
• Must have a general understanding of illnesses, disease processes, and medical conditions as they relate to the behavioral and emotional well-being of all patients
• Must have a thorough knowledge of documentation/record keeping requirements and social work standards
• Must have a reliable mode of transportation and possess a valid South Carolina driver’s license
