Details:

  • Under the direct supervision of the Behavioral Health Director, the Behavioral Health Care Advocate will provide a range of services that link behavioral health patients with necessary supportive services.  These may include internal services, as well as within the community.  This individual will serve as point of contact and liaison for external collateral contacts, to assist patients and their NHFHS’ care team (primary care with integrated behavioral health) in achieving optimal physical and mental health.  Assists Behavioral Health team with ongoing tracking of patients who are receiving behavioral health services, including phone contact, assisting with scheduling, particularly for brief interventions and crisis interventions. 

 

Experience and Skills:

Education:

  • Bachelor’s Degree from an Accredited School of Social Work

Knowledge and Skills:

  • Have minimum of three years in Case Management in Social Services/ Health Care Setting
  • Preferred to have five years in a Clinical Setting
  • Experience with accessing social support resources, working knowledge of the community, its systems and supportive resources
  • Able to advocate for patients and access to care/treatment
  • Communicate respectfully and professionally orally and written
  • Experience and solution-focused knowledge in working with individuals experiencing Behavioral Health issues such as SUDs, Mood Disorders, Anxiety Disorders including PTSD
  • Be comfortable with and able to provide empathic care for patients, ranging from pediatric to geriatric
  • Ability to provide compassionate, Trauma Informed (minimally Trauma Responsive) care to patients who may be experiencing chronic conditions such as chronic physical pain, homelessness, limited or impaired cognitive functioning
  • Must have a general understanding of illnesses, disease processes, and medical conditions as they relate to the behavioral and emotional well-being of all patients
